Athletic Paperwork and Physical Examinations

Dear parent/guardian,

It is the time to begin preparing for next year. On our website you will find all of the required District and UIL paperwork for your student athlete to participate in athletics. It is essential that you and your son/daughter read through this information and complete it thoroughly. It is also mandatory that all of these forms be on record with the athletic department prior to participation in athletics next year.

Physicals will be required for those student athletes entering into 7 th grade, 9 th grade, 11 th grade, and any new students to athletics/students not in good standing with current paperwork/students red flagged by the Athletic Trainer. Physicals are not required for students in good standing with current paperwork going into 8 th grade, 10 th grade, or 12 th grade.

Reminder , Check all pages for proper signatures and dates on the physical form and the rest of the packet. Your son/daughter will not be allowed to play or practice unless you have signed and completed all sections properly.
